Procedures for Requests for Disclosure of Retained Personal Data or Third-Party Provision Records

*If you wish to receive disclosure via electromagnetic means, please notify us. In principle, we will accommodate your request.

Procedures for Disclosure Requests

  1. 1. Upon receiving your request, we will mail you the designated request form ("Retained Personal Data Disclosure Request Form").
  2. 2. Please mail the completed form, documents confirming representative authority (if requested by a representative), and a postal money order for the processing fee (only for requests regarding notification of purpose of use or disclosure) to the Personal Information Desk listed above.
  3. 3. After receiving the request form, we will verify your identity by asking about approximately two items registered with us (e.g., phone number and date of birth).
  4. 4. Responses will, in principle, be provided to the applicant in writing (sent by post).

Documents Required to Confirm Representative Status

If the request is made by a representative, please enclose documents proving representative authority as well as identity verification documents for the representative. Registered domicile information in documents should be limited to the prefecture level; please black out any subsequent details. Also, ensure documents do not include My Number (Individual Number), or redact all digits.
